Emery Keelesdale Nurse Practitioner Led Clinic

Emery-Keelesdale Nurse Practitioner-Led Clinic was founded in 2010 as the first Nurse Practitioner-Led Clinic in Toronto and we are one of 25 clinics across all of Ontario. We have two clinic locations, providing comprehensive, interdisciplinary primary health care to our community.

Nurse Practitioner-Led Clinics (NPLCs) follow a primary health care model in which Nurse Practitioners are the lead health care providers. The Nurse Practitioners collaborate with other professionals such as registered nurses, dietitians, and social workers to deliver comprehensive and accessible services. Priorities also include enhanced health promotion, disease prevention and management, and providing care coordination to the community.
